Vera Art Studio Workshops Participant Waiver & Release Form

By signing this form, I acknowledge and agree to the following:

  1. I am voluntarily participating in a workshop hosted by Vera Art Studio.

  2. I understand that the activities may involve the use of tools, materials, or equipment that could cause minor injury if not used properly.

  3. I agree to follow all safety instructions and guidelines provided by the instructors or staff.

  4. I accept full responsibility for any personal injury, loss, or damage that may occur during or as a result of my participation.

  5. I release and hold harmless Vera Art Studio, its owners, instructors, and staff from any and all liability, claims, or demands arising from my participation in the workshop.

  6. I give permission for Vera Art Studio to take photos or videos during the event for promotional purposes, unless I explicitly opt out in writing.

  7. I confirm that I am in good health and capable of participating in the workshop.

  8. I understand that Vera Art Studio operates a no-refund policy. I agree that I am not eligible for a refund once I have attended the workshop, and I also acknowledge that if I fail to attend the workshop for any reason, I am still not entitled to a refund.
